12/9/2023 EnchantaVaughn Approved Upon arrival to the home setting, QMHPA opened up session with Ms. Broadnax and asked about any mental health emergencies at this time. Ms. Broadnax did not present any symptoms of SI or HI. QMHPA informed Ms. Broadnax that the purpose of this activity was to assist Ms. Broadnax with identifying appropriate ways to cope with depression. QMHPA assisted Ms. Broadnax with breathing techniques, exercising and guided imagery. QMHPA explained to Ms. Broadnax using appropriate coping skills daily will increase positive emotions. QMHPA engaged Ms. Broadnax in a second skills building activity. QMHPA explained to her that the purpose of this activity was to review with her techniques of handling anxiety when there is external stimulation. QMHPA guided Ms. Broadnax through a walking exercise where she is to identify relaxing situations to decrease depression. QMHPA explained to Ms. Broadnax about being able to display positive emotions to avoid depression by interacting with those throughout the community. QMHPA questioned Ms. Broadnax about interacting with family members and how she feels about being able to interact with family. QMHPA discussed with Ms. Broadnax reducing isolative behaviors. QMHPA explained to Ms. Broadnax isolation can cause depression to increase to do the lack of expressing feelings and bottled-up emotions. Ms. Broadnax appeared to be receptive to QMHPAâs interventions and services that were provided to her during the session. During the skills building activity, Ms. Broadnax was unfocused during breathing techniques and will laugh as if she was distracted. Ms. Broadnax was talking to herself and laughing out loud. Ms. Broadnax admitted to not using coping skills and will walk around as her only coping technique. Ms. Broadnax verbalized to QMHPA that she likes to walk near the water and feels relaxed. Ms. Broadnax said she use to work as a kid QMHPA and use to walk with the kids around the lake and this is why she knows a little bit of interventions to use. She indicated to QMHPA that she wish that she could apply those same techniques to herself at times but she doesnât know how to.
